myChart Proxy Access
What is Proxy Access?
Partial Access for Proxies
Proxy access allows parents, legal guardians, or caregivers to log into their own myChart account and connect to the health information of someone they are authorized to represent. Access levels depend on the patient's age and the appropriate authorization.
With partial access, proxies can view most of the patient's health information, except for sensitive content. They can also communicate with the care team.
Please note: Proxies with partial access cannot request prescription refills or view appointment details.
Proxies with full access can use myChart just like the patient. This includes viewing test results, sending messages to the doctor’s office, managing appointments, and requesting prescription refills.

Proxy Access by Age Group
For Patients 11 and Under
For Patients Ages 12 to 17
For Patients 18 and Older
Partial Access: Not available for this age group
Full Access: Granted to parents and legal guardians
Default Setting: Proxy access is limited to protect the patient’s privacy. Restrictions begin once the patient turns 12.
Full Access: Can be granted with consent from the patient and approval from their care provider. Access is reviewed each year and ends on the patient’s 18th birthday.
Partial Access: Not available for this age group
Adult-to-Adult Proxy Access: Allowed with the patient's consent. The patient must authorize access for another adult to view and manage their health information.

3. Teen Proxy – Enhanced
Applies To: Children under the age of 12
Access Level: Full access to the child’s myChart account
How to Get Access: Complete and submit the myChart Proxy Form to the child’s care provider
Ends When: Automatically converts to Teen Proxy with limited access on the child’s 12th birthday
Applies To: Patients aged 12 to 17
Access Level: Limited access to the patient’s myChart account
How to Get Access: Submit the myChart Proxy Form to the patient’s care provider
Ends When: Automatically terminates on the patient’s 18th birthday
Applies To: Patients aged 12 to 17
Access Level: Full access to the patient’s myChart account
How to Get Access:
Teen must give consent
Both teen and guardian must complete the myChart Proxy Form
Submit form to the patient’s care provider
Ends When: Automatically terminates on the patient’s 18th birthday
4. Adult Proxy
Applies To: Adults 18 years and older
Access Level: Full access to the adult patient’s myChart account
How to Get Access:
Patient sends an online or in-person invitation
Both patient and proxy complete the myChart Proxy Form and submit it
Ends When:
Patient submits a request to end access
A specified expiration date is reached
Patient revokes access from within their myChart account
